$30 BY MAIL. Payment must accompany entry form and be postmarked on or by November 27, 2011.

Family rates for 3 or more are 2x Single registration plus $10 for each player after the first 2.
Proof of valid national membership must be presented by all participants - NO EXCEPTIONS
